What Is a Daycare Illness Form? 

A daycare illness form is a printable form completed at the daycare by an official childcare provider and sent home with a child if they display signs of an illness or infection. 

The form is an effective way to communicate with parents about their child's symptoms. The form also indicates when the child can return to the daycare. 

This form can also be used as a valid excuse for parents who need to miss work to look after their sick child.

When to Send a Daycare Illness Form  

If not appropriately monitored, infections and illnesses in daycare can spread quickly, resulting in more than one ill child. But how do you know when to send a child home with a daycare sick note? 

Depending on what communicable disease the child in your care has, you may see different symptoms. Things like runny noses, fevers and sore throats often spread to other children throughout a daycare center. 

Running a fever is undoubtedly a time when a child needs to be sent home as soon as possible to avoid passing their illness to other children. However, you may also choose to request that parents get their child tested for COVID-19 if the symptoms are severe enough. 

 

What to Include on an Illness Form 

A printable daycare illness form is a way to let parents know their child is sick and an excellent way to remind them of any daycare sick policies you have in your daycare. This makes it an essential business tool for your daycare. 

Use the form to record information such as the child's name and their symptoms, and the date and time you noticed the child's illness. The form should also include any policy reminders you want parents to remember.
